Cancer questions for survey?

Okay, so I'm doing a Science Fair project on cancer and I need about 10 questions that I will be asking people in my school for a survey. It'll be a survey like for risk factors on cancer; how many people are more likely to have cancer than others, something like that.Example, one question I will be asking would be "Do you live with someone who smokes?"
So can someone give me some questions that I can ask? Thanks!

1. do you have any relatives who have cancer?
2. how many times a week do you exercise?
3. do you spend a lot of time in the sun?
4. do you work with chemicals a lot?
5. do you think you have a healthy diets?
6. something along the lines of alchohol...(idk)

1. Do you smoke (either pot or marijuana)? If so, how many cigarettes/how much? And how long have you been smoking? If not, do you live with someone who smokes? How long have you lived with them?
2. How many times have you gotten sunburned in your life?
3. Has anyone in your family died from cancer?
4. Do you have light skin, light eyes, and light hair?
5. Have you ever been exposed to Abestos or Benzene for a long period of time?
6. Are you overweight? [exercising regularly can help prevent cancer]
7. Have you ever been tested for HPV? Have you ever been diagnosed with it? If you've never been tested, have you slept with someone without a condom? [HPV is the leading cause of cervical cancer in women]
8. Do you drink alcohol a lot? [alcohol can increase the risk of getting certain cancers--ex. Women who have 1 drink a day have a very small increased risk. Those who have 2 to 5 drinks daily have about 1½ times the risk of women who drink no alcohol]
9. What is your race? [White women are slightly more likely to get breast cancer than are African-American women. But African American women are more likely to die of this cancer. At least part of the reason seems to be because African-American women have faster growing tumors. Asian, Hispanic, and American Indian women have a lower risk of getting breast cancer.]
10. Do you eat a high-fat diet? [a low-fat diet can help prevent cancer]
