Community service project?

I'm doing a service project tonight and I want to know how I get credit for it. Do you just write down what you did on any piece of paper and have a supervisor sign it or is there a specific piece of paper you have to record it on. I asked my school and they told me to find the service club teacher but the teacher in charge of the club wasn't there.
