My community service project question?

I am going out and collecting people books in order to donate them to the poor. I also made a sheet which asks the donor to give their name and phone number. Should I tell them to sign their name or just write their name?

I would have the write their name and sign next to it if you are going to need to read the names later. It might be nice to send out thank you cards or something like that and it would be much easier to do if you could read their name. Good Luck!
