logging community service hours?

i need to start logging my community service hours, because i need a certain number of hours for national honor society that i just got accepted to. but how do you do it?
can i keep a small journal to put in hours and the signatures of the people in charge? or do i need to get a sheet from somehwere? my friend joined the key club and she has key club's own comm. service hour sheet.
not only im doing this for nhs, but i just wanna keep a record of it myself.
can i put it in anywhere as long as i get the signatures and get it organized?
please help! thank you
