Family Issues At Age 14- Stressing Me Out: What to do?

Hi,
I'm 14 years old, make High Honors, and I am disabled. I've a hard time making friends because of my disability. But, if that's not bad enough, I've my home life. I live with my grandparents who are in there sixties. My mom doesn't come around that often, I see her once- twice a week if that. I feel like I'm imposing on my family because they always need to meet my physical needs no matter what, no matter how tired they are. Then, I have a 12 year old brother who is going on 13. He's always messing around, getting phone calls home from school. My gram has tried talking to him about it, trying to find out why he does it, he says that he just doesn't think before he acts. She has tried taking stuff away- TV, stuff like that. Then, I have a grandfather who has heart problems. He's had three heart attacks throughout his life. He doesn't do anything as simple as preparing dinner or putting away clothes to healp my gram out. Then, I have my uncle who sits at my house and watches TV.
My uncle won't help out, we are short on money, and have a hard time making ends meet. My dad is in IL on business hoping to make things right, and my family more money. I can't walk, and that bugs me very much.
