Help with info on Heart Disease please?

I am doing a report on Heart Disease for my anatomy and physiology class. I have to pretend that I am a spokes person for the disease and create a pamphlet. So first could you guys give me some great websites about heart disease?
Second and this is big: Once you are at high risk of heart disease and your arteries are blocked and so on, you’re a smoker and over weight and have a bad diet but you have no symptoms, you have only been told you have a bad heart. Can you loose weight, stop smoking exercise more and have a much better diet and get rid of that build up on your hear naturally? Can someone please help me with that and kindly provide a website saying so? I would be so grateful!!! (I know it can be prevented, but once you get the heart disease can you work on it going away?)
