how do I obtain a copy of my medical history?

I have a potential employer that is requesting a copy of my medical history...the problem with this is since I don't have insurance I go to free clinics here and there for check-ups and etc. is there a national database where my all my medical information is stored so that I and/or my potential employer can obtain a copy? is so where can I find this or who can I call?

go to the last place where you went to and request a copy of your history. it will probably cost you a few bucks and days, but they have to give it to you.

even though its incomplete, if you answered all the questions truthfully, that record will have a basic medical history on you.

yes and no, an employer can ask for your medical history, and depending on the job you may have to turn it over. Ask to talk to the HR person and ask why the company needs to see them

A government position, then yes you have to submit a medical history. If you say no they will turn down your application immediately. Even though this is personal information, your health status for your job could be important and if there is any known condition they should be aware of.

I doubt there is a national database. I have never heard of one. They do need to get one though.

I don't think your employer should be asking for your medical history anyways.

Your medical history is yours, so you may have a copy of what's available for the asking and perhaps a small copying fee.
Hopefully, there is not now and never will be a 'national medical database'. For the moment, you must go to the various medical services providers to get what histories each has.
A word of caution: be very careful to whom you release these medical files. The information in them can cause you great trouble in the wrong hands.
