How do I circumvent the school's web filter using my home ip address?

I heard there was a way that you can leave your computer at home on, some how use your computer's ip address on the school computer and access any site at school and apparently it works for any filter ever built, and every filter that is going to be built, until the home ip address is blocked. Please help me! I know it's possible!
