How do I place a dinner reservation over the phone in Japanese?

I'm going to try and call a restaurant in Tokyo from the U.S. My Japanese is pretty bad, I kind of know what to say, but my sentence structure is awful. I want to place it for Monday March 31st at 6:30pm.
