doing medicine in the US.?

I want to study medicine here in the US but i want to know if the medical graduate here can get any job after graduation.Can any hospital employ him as a general consulatant of much higer nurse and pay him?Secondly how is residency in medical school achieved?If i go to school A in another state,finish my undergraduate and go to the same school A*s medical school,will they give me residence tuition since i did both undergraduate and graduate medical schooling in the same school and state.3rdly,when one is told to specialise in a field like surgey,gynae etc will that fellow be paid something whiles working and acting on patients?for specialization,will the student have to go to another schooling again and pay tuition?4 questions in all,please help ,me fellow doctors and medical students.
