Kentucky Pain Clinics?

i live in a small town where pain med abuse is comon in younger people. i hurt myself in an atv accident several years ago and since then have had back pain problems but the docs say that my back is fine. and cant do anything about it. i fell down the steps around a year and a half ago and didnt go to the doc until like six months later and the xrays were still normal. no damage but nothing explains this pain that i experience, i cant sleep unless i have a heating pad on my back, i cant sit in a car for longer than an hour or my knees hurt bad. the docs wont do anything, do I have to be referred to a pain clinic? can I request a referral? never had an mri. i dont knwo if that would show a difference. can I just walk in a pain clinic and request one of the docs there to look at it?
