What are your opinions of a man cohabitating w/woman, and no action after 5 months?

Am kinda concerned that the woman is nieve about the man having serious issues. He's told her he has a secret but won't tell what it is. I've known this person since she was in the 5th grade. Am I wrong to tell her to have him expose the secret?
It's supposed to be a relationship. She's 33 and he's 40.
