What vaccine exemption document do I need to give my son's preschool in Georgia...?

to allow him to continue to go to school without being vaccinated? Does it need to be notarized? Does a doctor give it to me? I appreciate any help if you've been through it and have the answer!

I don't know about the state of Georgia, but here in Utah you have to get something from the health department. All it was was a piece of paper that basically stated you were opting out of vaccinations. It didn't need to be notarized, but it did need to be signed.

If you aren't sure where to get one, call your child's drs office, the health department or the preschool. Someone should be able to help you.

By the way, don't let the people who tell you to vaccinate your kids get to you. Each parent should be allowed to choose.
