When do you tell someone you are dating about a health issue - not sexual?

I have Cardiomyopathy with congestive heart failure. With meds I am able to keep it in remission for now. It is a progressive disease. I have a pacemaker / defibulator. I am only 43 years old. When do you tell someone you are dating?
