Heroine is an extremely effective pain killer for those with severe injury or terminal illness. Unfortunately, it's so addictive it had to be banned.
Opium is highly effective in treating chronic diarhea. Back before water purification was common this was a major problem. A person could dehydrate and die within a week. Opium saved many lives. Unfortunately, due to its highly addictive nature it now has to be extremely controlled.
