Im looking for a software where I can program a test or question and answer?

I have a health evaluation form and it has questions about a persons diet and they can select a yes or no box. For example: Do you exercise? Yes, No. What I want to do is put this paper form into a electronic form and have it grade you and give you a score after they are finished.
