Why are STDs condidered "public health" when sex is considered a private affair?

Why are STDs condidered "public health" when sex is considered a private affair?

I mean, how can someone get an STD unless they are only taking part in private affairs. "Public sex" is legal nowhere exept in The Netherlands; right?
